If you change name, address or telephone number, please let our receptionists know by calling or writing to us. Alternatively you can complete the online form below. If you move outside the practice area you may need to find a doctor in your new area.
Any changes to your registration details must be notified to the practice.
If you move to a new address
If your new address is outside of the practice area you will be asked to register with a practice in your new locality. It sometimes comes as a surprise for patients who move only a small distance to be asked to do this – if it is important to you, you are advised to check with the practice that your new address is within the practice area. Patients who change address without notifying the practice will be de-registered and their records forwarded to Cheshire Health Agency.
