REGISTER?

Patients wishing to join the practice should bring along their medical cards to reception for registration. The details on your card will help you complete the registration form (you must provide all of the information requested). Any changes to your registration details must be notified to the practice as soon as possible.

If you change your address and your new address is outside of the practice area you will be asked to register with a practice in your new locality. It sometimes comes as a surprise for patients who move only a small distance to be asked to do this - if it is important to you, you are advised to check with the practice that your new address is within the practice area. Although the practice boundaries are shown on the map on the Practice Area page, this is only a guide. Our reception staff will be able to confirm whether your new address is within the area if you give them the exact address.

Patients who change address without notifying the practice will be de-registered and their records forwarded to Cheshire Health Agency.
